  • Below 19 points: the fall risk is high, and a multifactorial approach is required.

A point of caution: unlike some geriatric scales, a high score on the Tinetti indicates a good result. The inversion of logic compared to other grids (such as some alternative versions rated out of 35 where a high score indicates a high risk) regularly creates confusion. Always check the version used before interpreting.

The recommended logic starts with simple questions: has the patient fallen in the last twelve months? Do they feel unstable while walking? If the answers indicate a risk, a mobility test like the Tinetti or the Timed Up and Go (TUG) clarifies the level. The TUG, which times the duration needed to stand up, walk three meters, turn around, and sit back down, provides a temporal data point that the Tinetti does not offer.

Both tests evaluate similar but not identical dimensions. The Tinetti provides a detailed item-by-item breakdown useful for targeting rehabilitation: it identifies whether the problem lies in step initiation, symmetry, pivoting, or standing stability. The TUG gives a quicker overall result, suitable for screening in general medicine consultations.

Beyond the motor test, the multifactorial assessment includes a review of medications (psychotropics, antihypertensives, and polypharmacy increase risk), searching for orthostatic hypotension, assessing frailty and sarcopenia. An isolated Tinetti score is insufficient to build a prevention plan.

Contexts for using the Tinetti test: nursing homes, home, and rehabilitation

The test is not limited to a single care setting. Its printable paper format makes it particularly suitable for situations where access to a digital tool is limited.

In nursing homes, the Tinetti often serves as a reference during admission and then during regular reevaluations. Comparing scores over time allows for the detection of a gradual decline in mobility before a fall occurs.

In physiotherapy practices, the item breakdown guides the rehabilitation program. A patient who loses points on the pivoting and standing stability items with eyes closed will not work on the same exercises as a patient whose deficits are related to step length and height.
